Who: Marching and color guard students only

How: Please print and sign your completed forms; we will collect them on August 1st before the start of summer camp. We must have all your forms on file before you can participate in camp.

Some of the forms have fillable fields. To keep an electronic copy of your filled-out forms, you will need to download the forms before filling them out. If you don’t need an electronic copy, simply open them, fill them out, and print them immediately.

• FCPS Medication Authorization Form for Over-The-Counter Medications (optional, but highly recommended)

  • This form allows us to administer common over-the-counter medications during band functions; without this form on file, we cannot provide your student with any medications, even Tylenol.

  • You can cross out any medications you do not want your student to be given.

  • It does NOT require a doctor's signature.

• FCPS Medical Authorization Form for Prescribed Medications (optional)

  • This form allows us to administer prescribed medications during band functions.

  • It must be signed by a doctor.

• FCPS Epinephrine Authorization Form (optional)​

  • This form allows certified personnel to administer an epi-pen during band functions.​​

  • It must be signed by a doctor.

​​

• FCPS Inhaler Authorization Form (optional)​

  • This form allows your student to administer a rescue inhaler during band functions.

  • It must be signed by a doctor.

• Virginia High School League Pre-Participation Physical Exam Form (optional, but highly recommended)

  • This form allows your student to be seen by the FHS athletic trainers in case of illness or injury while on school grounds.​​

  • It must be signed by a doctor (requires a current physical).

  • Physicals can be completed by your child's doctor or at an FHS-sponsored Physical Night, which typically occurs in May for the following year.
