2020/21 Band Forms and Fees
Please follow the steps below to sign up for the 2020/21 Fairfax Bands program. Students participating only in the concert bands (ie. taking band as a class) should complete STEP 1. Marching and color guard students must complete STEPS 1-3.
STEP 1: Register for the Band Program (required)
Who: All band students (concert bands, marching band, color guard)
STEP 2: Complete FCPS Forms (to be turned in this summer - time TBD)
Who: Marching and color guard students only
Fill out the following forms, print and turn them in this summer (date/time TBD)
Some of the forms have fillable fields. Certain browsers allow you to fill in the fields online and then print the form. For those that do not, save the form to your computer first, then fill/print.

Marching contract (required)

FCPS Medication Authorization Form for Over-The-Counter Medications (optional, but highly recommended)

This form allows us to administer common over-the-counter medications during band functions; without this form on file, we cannot provide your student with any medications, even Tylenol.

You can cross out any medications you do not want your student to be given.

It does NOT require a doctor's signature.

FCPS Medical Authorization Form for Prescribed Medications (if needed)

This form allow us to administer prescribed medications during band functions.

It must be signed by a doctor.

FCPS Epinephrine Authorization Form (if needed)

This form allows certified personnel to administer an epi-pen during band functions.

It must be signed by a doctor.

FCPS Inhaler Authorization Form (if needed)

This form allows your student to administer a rescue inhaler during band functions.

It must be signed by a doctor.

Virginia High School League Pre-Participation Physical Exam Form (optional, but highly recommended)

This form allows your student to be seen by the FHS athletic trainers in case of illness or injury while on school grounds.

It must be signed by a doctor (requires a current physical).

Physicals can be completed by your child's doctor or at an FHS-sponsored Physical Night, which typically occurs in May for the following year.

STEP 3: Pay Marching Fees by the first day of Rookie camp - July 29, 2020 (required)
Who: Marching and color guard students only
Anyone who has registered for our 2020/21 marching season will be notified when the payment portal is up and ready to receive funds (typically early July) for the following:

Drillmaster shoes (if needed)

Additional show shirts (if desired)
Students who qualify for Free and Reduced Lunch should contact Patric Buchroeder once payment collection starts to find out what your marching fees will be and how to pay them.
